9-29-2011 Seasonal flu shots (for 2011) are available NOW to ALL children ages 6 months

to age 18 years, established patients only!

Sorry, we do NOT provide seasonal flu shots for adults (older than 18 years old).

9-29-2011 On December 1st 2011, MFM will begin charging a $65.00 fee for any missed

appointments. You can avoid this fee by calling us AT LEAST 24 HOURS

IN ADVANCE of your scheduled appointment to cancel your visit.

9-29-2011 PHYSICIANS AND DENTISTS CREDIT BUREAU INC. will manage our

seriously past due accounts (those accounts with balances that have been due

for over 90 days with no payments made).

They will provide a means for setting-up payment plans and for collecting the

remainder of account past due amounts.
